Chorus Survey - 4th & 5th Grade

Dear GREEN Parents,

Please use this form to express interest in our 4th and 5th Grade Chorus. If you have more than one child interested, please fill out a separate form for each student. Depending on interest, students may need to sing a simple song for Mrs. Bloom. More information will be sent out regarding further instructions.

Chorus will rehearse on Tuesday afternoons from 3:30-4:30 pm. Our first rehearsal will be Tuesday, September 20th. Please double check your family calendars to make sure you do not have any scheduling conflicts.
